Proof of Residency– You MUST be a St. Johns County resident to enroll your child. Immunizations– All immunization records must be on the Florida Certification of Immunization, DOH 680 Form (Florida State Statute 1003.22). For more information on immunization requirements, please view the Immunization Guidelines.
